These documents govern your relationship with Dr. Eve Fontanella Mobile Veterinary Services. You will be asked to sign a combined agreement at your first visit.

By accessing or using the services of Dr. Eve Fontanella Mobile Veterinary Services ("Practice," "we," "us," or "our"), including this website, submitting an intake form, or scheduling a veterinary appointment, you agree to be bound by these Terms of Service. If you do not agree, please do not use our services.

These Terms are effective as of July 1, 2026 and apply to all clients, pet owners, and authorized agents who interact with the Practice.

Dr. Eve Fontanella Mobile Veterinary Services provides mobile veterinary care to pets in the Saratoga Springs, New York area. Services include but are not limited to: preventive care, wellness examinations, pain management, hospice and palliative care, diagnostics, and supportive treatments.

This practice is not an emergency facility. We do not provide emergency veterinary services. If your pet is experiencing a medical emergency, you must contact an emergency veterinary hospital immediately. The Practice shall not be liable for outcomes resulting from failure to seek emergency care.

Services are available on scheduled appointment days only. Appointment availability is not guaranteed and is subject to change.

A valid Veterinary Client Patient Relationship (VCPR) must be established before the Practice can provide medical recommendations, prescriptions, or ongoing treatment. A VCPR is established when:

  • Dr. Fontanella has conducted a physical examination of your pet;
  • She has assumed responsibility for making medical judgments regarding your pet's health;
  • You, the client, have agreed to follow Dr. Fontanella's instructions.

A VCPR may expire if the Practice has not seen your pet within a reasonable period. Renewal may require a new examination.

Appointments are requested through the online intake form or by direct communication with the Practice. Submission of an intake form does not constitute a confirmed appointment. The Practice will contact you to confirm date, time, and availability.

We ask that you provide at least 48 hours' notice if you need to cancel or reschedule. Repeated late cancellations or no-shows may result in a cancellation fee or discontinuation of services at the Practice's discretion.

Any required deposits are subject to the Practice's deposit and refund policy communicated at the time of booking.

You agree to:

  • Provide accurate, complete, and current information about yourself and your pet;
  • Ensure a safe environment for Dr. Fontanella during home visits, including restraining other animals and disclosing any hazards;
  • Be present (or have an authorized adult present) during the veterinary visit;
  • Communicate any changes in your pet's condition, medications, or history promptly;
  • Pay for services rendered in accordance with the Practice's fee schedule;
  • Notify the Practice if you are under 18 years of age, as a parent or legal guardian must consent to services.

Payment is due at the time services are rendered unless prior arrangements have been made. The Practice accepts payment by credit card, debit card, check, or cash as specified at the time of booking.

Costs for recommended diagnostics, treatments, medications, and procedures will be discussed with you prior to being performed. You will not be charged for any service without your explicit verbal or written authorization.

Outstanding balances may be subject to a late fee and/or referral to a collections agency. The Practice reserves the right to decline future services until outstanding balances are resolved.

To the fullest extent permitted by applicable law, the Practice and Dr. Fontanella shall not be liable for any indirect, incidental, consequential, or punitive damages arising from the provision of veterinary services, including adverse reactions to treatments, complications from pre-existing conditions, or outcomes of recommended or declined procedures.

The Practice's total liability for any claim arising from veterinary services shall not exceed the total fees paid for the specific service giving rise to the claim.

Nothing in these Terms limits liability for gross negligence, willful misconduct, or fraud.

These Terms are governed by the laws of the State of New York. Any disputes arising under these Terms shall be resolved in the courts of Saratoga County, New York, and you consent to such jurisdiction.

The Practice reserves the right to update these Terms at any time. Material changes will be posted on this website with an updated effective date. Continued use of our services after such changes constitutes your acceptance of the new Terms.
