Request a Visit
Complete our simple online visit request. Tell us about your pet, your location, and what you're looking for. Dr. Fontanella personally reviews every request.
We Review Your Request
Dr. Fontanella reviews your information and reaches out to discuss your pet's needs, confirm availability, and answer any questions. This is not a confirmed appointment until she personally confirms.
Complete Secure Digitail Intake
After review, you'll receive a secure link to complete your full intake — medical history, agreements, vaccine records, and consent forms — all through Digitail, our secure veterinary platform.
Schedule & Pay Securely
Once intake is complete, your appointment is confirmed and scheduling and payment are finalized through Digitail. A deposit may be required for certain visit types.
Receive Care at Home
Dr. Fontanella arrives at your home at the scheduled time. Your pet receives thoughtful, unhurried care in the comfort of their own environment — no waiting rooms, no stress.
Is my request a confirmed appointment?
No — submitting a request is the first step. Dr. Fontanella will personally review and confirm before any appointment is scheduled.
What if I'm outside the service area?
You're still welcome to submit. Requests slightly outside the standard area may require a travel fee or may not be able to be accommodated. Dr. Fontanella will review personally.
Why do I use Digitail for intake?
Digitail is a secure veterinary platform that protects your pet's medical records, agreements, and payment information. It's used by many veterinary practices for safe, HIPAA-conscious record-keeping.
What if it's an emergency?
Please do not wait for a callback. Contact a 24-hour emergency veterinary hospital immediately. Dr. Fontanella's service is scheduled, non-emergency care only.
