Dr. Eve offers two ways to get started. An in-person visit establishes a veterinarian-client-patient relationship so she can provide ongoing care. If you're not ready for that yet, a virtual consultation is a great way to ask questions and learn more.

This form is not monitored for emergencies. If your pet is having trouble breathing, collapsing, unable to urinate, having seizures, bleeding heavily, showing signs of severe pain, or has ingested a toxin, please contact or go directly to an emergency veterinary hospital.

Please do not include urgent medical concerns, detailed medical history, lab records, vaccine records, or photos in this form. For visit requests, use Request a Visit.
